Chronic Disease Management Resource for General Practice

The Allied Health Professions Australia’s ABHI funded CDM project is now finalised. They have a new website with resources and information about referring people with chronic disease to allied health professionals. The website covers those allied professionals that are Medicare rebateable. A limited number of hard copies of the resources will be distributed in the next month. A media release (PDF) is attached for your information.
