Mental Health Nurse Service – Patient Information

What is the Mental Health Nurse Service?

The Mental Health Nurse Service works closely with Psychiatrists and/or General Practitioners to provide assessment, coordinated care, treatment and support for people with a severe mental health condition.

Who is eligible for this service?

The Mental Health Nurse Service is for people who:

• Have a mental illness who are currently receiving treatment for their condition
• Have a mental illness that affects the way they live (i.e. occupationally, socially and emotionally)
• Are concerned about their mental health illness and/or have previously been an inpatient in a Psychiatric facility
• Need additional support to assist in managing their illness and treatment plan
• Need help to link into other community services  

How can the service support you?

Patients will receive:

• Free sessions with a Mental Health Nurse
• Regular support so you can explore ways of managing your illness
• Help in identifying early warning signs that you may become unwell and strategies as to how you can manage this situation

 Referral and Appointments

Referral can only occur via your GP.  The Mental Health Nurse will see you at the GP clinic 

Review and Exit

Patients will be reviewed every three months.  A patient will no longer be eligible for the service when they no longer require the clinical services of a mental health nurse or they become case managed by Latrobe Regional Hospital Mental Health Services

* Please note: If you are under the care of Latrobe Regional Hospital, Mental Health Services you can not access this service.
